USA Concusion Injury Management Form
The USA Soccer Club has developed a form for use by team coaches to manage the process related to a potential concussive head injury. The form contains is a step-wise process meant to guide our coaches through the identification, parent notification, action, clearance to play, and club submission processes.

MSYSA Concussion Awareness Form
Read and download the MSYSA Concussion Awareness Form that is required for for participation on a team.
MSYSA Concussion Cleared to Play Form
If a player has been sidelined with a concussion and has now been cleared to play by a medical profession, the player must read and download the MSYSA Concussion Cleared to Play Form prior to rejoining the team for training or play.
